Please complete the forms below and deliver them (by mail, fax or in person) to the school where you wish to become a mentor. If you do not have a preference, we invite you to deliver the paperwork to your nearest school and/or the District Office, 118 South Sixth Street, PO Box 4010, Odessa, DE 19730-4010. Upon receipt, you will be contacted by a mentoring coordinator to discuss the program and any questions you may have.
